Military Survivor Benefits Plan

Kay is divorcing a member of the military and is confused about whether or not to sign a form "DD 2656."

Question:

My question is how to get SBP coverage. I am at the final stages of getting the divorce decree on paper but not sure of SBP coverage. My spouse wants me to sign a form (dd 2656). He tried to get me to sign this early on in the separation, which I would not because it had nothing on it about me. Does anyone know anything about this form what it should state? I am going to get 30% of his retirement and he has taken a job now that he retires that will require him to fly a lot.

Any input would be helpful. Thanks
